Poverty and lack of decent jobs suck people in Herat Province, Afghanistan, into a vicious circle.

They sell their kidneys to raise money to pay off debts, but poorly performed surgeries and possible infections leave them impaired and unable to hold a steady job to earn more money once the savings run out.
